The Issue at Completion
A major Client with a 2-billion-pound turnover is in the business of providing "built" solutions to technical issues within the industry. They have an abundance of procedures and half a dozen hold points to review the progress of developing these solutions towards build and financial commitment. A board of Directors are involved at each key review stage. At completion of the build - and after "end-user" use, the project is reviewed. What is a common theme to the output from this review? "The objectives set at the start were not fully developed and understood by the team."

PDCA 
William Edwards Deming (October 14, 1900 – December 20, 1993) was an American engineer, statistician, professor, author, lecturer, and management consultant. Educated initially as an electrical engineer and later specialising in mathematical physics, he helped develop the sampling techniques still used by the U.S. Department of the Census and the Bureau of Labour Statistics. In his book, The New Economics for Industry, Government, and Education, Deming championed the work of Walter Shewhart, including statistical process control, operational definitions, and what Deming called the "Shewhart Cycle "which had evolved into Plan-Do-Check-Act (PDCA)
The Deming Cycle
Plan-Do-Check-Act means that whether solving a problem or building a plan everyone should follow this process to ensure learning and success towards the next goal. This cycle of "Plan - Do - Check - Act" is also known as the Control Circle, or simply PDCA.
As part of Japan's reconstruction efforts, they sought an expert to teach statistical control. From June–August 1950, Deming trained hundreds of engineers, managers, and scholars in the concepts of quality. He also conducted at least one session for top management (including top Japanese industrialists of the likes of Akio Morita, the cofounder of Sony Corp - Deming's message to Japan's chief executives was that improving quality would reduce expenses
Kaoru Ishikawa expanded Deming's four steps into six:
1. Determine goals and targets.
2. Determine methods of reaching goals.
3. Engage in education and training.
4. Implement work.
5. Check the effects of implementation.
6. Take appropriate action.
